Pre- Entrance Health Record
Virginia State University requires physical examinations for all first time enrollees (freshman, transfer and graduate students) and for their health records to be on file in SHS prior to registering for classes.  Any student who cannot produce an up-to-date immunization record must be reimmunized at his/her expense.  Registration cannot be completed until an up-to-date immunization record is provided. Also, all students must have their last tetanus shot on file in case of  an injury. It must be understood that immunization records are part of the health record and they must be up to date to comply with Virginia State Law. You WILL NOT be able to register for any classes if your health record is not up to date or if it is not on file in the SHS office.

Immunizations are given at the office of SHS for a FEE.  Menomune A/C/Y/W - 135 ($80.00), Tetanus/Diphtheria ($18.00) PPD ($15.00) and MMR ($40.00)

Hours of Operation
The office of Student Health Service is open from 8:a.m. to 5:00 p.m. Monday through Friday. There is no charge for clinic or office visits,  however, to be seen in the clinic you must have your student ID. An appointment system has been established to ensure that students  are seen in a timely manner, however no sick student is turned away.  If you are willing to wait you will be seen the day you come in. If you are unable to make a scheduled appointment please be sure to cancel the appointment so that other students can be seen in a timely manner.


Emergencies
In case of an emergency students should contact security at 524-5411. This number is a direct line to the Campus Police Emergency line.


Medical Emergencies 
If you should become ill after hours, students are advised to use Health Care Plus for non- emergency situations that cannot wait until SHS opens again. Health Care Plus is open from 8:00 a.m.- 8 :00 p.m..  This is encouraged rather than using the Southside Regional Medical Center emergency room because you will only be charged for an office visit rather than an emergency room visit. Should you need to get to Health Care Plus resident educators have cab vouchers available for your use.


Class Excuses
Excuses for missed classes are only given to students who have been served during their illnesses or upon the discretion of the clinician.  Excuses will only be given for legitimate complaints, such as fever, infection, severe pain, etc.  Excuses will not be given during the midterm or final examination period.


Confidentiality
To protect student's privacy, the Student Health Services does not release information contained in the record to anyone without a student's written consent or a court order.
