Changing Majors Process/Checklist

Please pay close attention to all deadlines on applications and forms.

• Application for Admission into Teacher Education **
• Admission Application: Part C (faculty advisor should download)
• Admission Application: Part D (faculty member should download)

• Field-Based and Clinical Instruction Handbook (includes Evaluation Form and Attendance Record)
• Field Experience Evaluation Form and Attendance Record

• Student Teaching Application Packet*
• Assessment of Student Teaching
• Assessment of Student Teaching Rubric
• Student Teaching Internship Handbook
• Cooperating Teacher Handbook

* These forms must be word processed. Save them as a new document on your hard drive before completing them.

** Parts A and B must be word processed.
